(피렌체, 이탈리아 2021년 1월 22일 PRNewswire=연합뉴스) 21일, 이탈리아의 민간 제약 및 진단기업 Menarini Group이 엘존리스(ELZONRIS)(tagraxofusp)에 대해 유럽위원회(European Commission, EC)로부터 성인 모구형질세포양수지상세포종양(Blastic Plasmacytoid Dendritic Cell Neoplasm, BPDCN) 환자의 일선 치료용 단일 요법으로 마케팅 승인을 받았다고 발표했다. BPDCN은 예후가 좋지 않은 희귀 진행성 혈액암 종양이다. EC의 이번 결정 전인 2020년 11월에 유럽 의약품청(European Medicines Agency, EMA) 산하 약물사용자문위원회(Committee for Medicinal Products for Human Use, CHMP)로부터 긍정 의견을 받은 바 있다. 이번 결정은 치료를 받은 적이 없는 환자와 BPDCN 치료를 받은 환자를 대상으로 진행된 최대 규모의 전향적 임상시험 결과를 바탕으로 한다.

모구형질세포양수지상세포종양(Blastic Plasmacytoid Dendritic Cell Neoplasm, BPDCN)

CD123

경과와 주의

- 생명을 위협하는 치명적인 사례를 포함하는 모세혈관 누출 증후군(Capillary leak syndrome, CLS)의 경우, 치료 주기의 첫 5일에서 가장 많이 발생한다. 치료를 시작하기 전에 환자의 심장 기능이 적절한지, 혈청 알부민이 3.2g/dL 이상인지 확인해야 한다. 치료 중 각 용량을 복용하기 전에, 또는 임상 지시보다 더 자주 혈청 알부민 수치를 모니터링해야 한다. 그뿐만 아니라, 환자가 CLS의 다른 징후나 증상을 보이는지 파악해야 한다. 환자에게 CLS 증상 여부, 그리고 언제 즉각적인 의료적 처치를 받아야 하는지를 알려야 한다. 정맥 알부민 보충과 복용 중단이 필요할 수도 있다.

- 엘존리스로 치료한 환자에서 심각한 과민증 반응이 보고됐다.

- 엘존리스로 치료한 환자 중 20% 이상에서 발생한 부작용으로는 저알부민혈, 아미노 전이 효소 증가, 혈소판 감소증, 구토, 피로 및 발열 등이 있다.

- 이상 사례 공통용어기준(Common Terminology Criteria for Adverse events, CTCAE)에서 3급 이상이며, 환자의 5% 이상에서 발생한 부작용은 아미노 전이 효소 증가, 혈소판 감소증 및 빈혈이었다.

Menarini Receives European Commission Approval of ELZONRIS (tagraxofusp), for the Treatment of Blastic Plasmacytoid Dendritic Cell Neoplasm (BPDCN)

FLORENCE, Italy, Jan. 21, 2021 /PRNewswire/ -- The Menarini Group, a privately held Italian pharmaceutical and diagnostics company, announced today that the European Commission (EC) has granted a marketing authorization, for ELZONRIS (tagraxofusp) as monotherapy for the first-line treatment of adult patients with blastic plasmacytoid dendritic cell neoplasm (BPDCN), a rare, aggressive hematologic malignancy with dismal outcomes. The EC decision follows the positive opinion received from the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) in November 2020, and is based on the results of the largest prospective clinical trial ever conducted in patients with treatment-naive or previously-treated BPDCN.

ELZONRIS has been granted orphan designation in Europe, and is now the first approved treatment for patients with BPDCN, and the first approved CD123-targeted therapy in Europe to address this high unmet medical need.

About BPDCN

BPDCN, formerly blastic NK-cell lymphoma, is an aggressive hematologic malignancy, often with cutaneous manifestations, with historically poor outcomes. BPDCN typically presents in the bone marrow and/or skin and may also involve lymph nodes and viscera. The BPDCN cell of origin is the plasmacytoid dendritic cell (pDC) precursor. About CD123

CD123 is a cell surface target expressed on a wide range of malignancies including blastic plasmacytoid dendritic cell neoplasm (BPDCN), certain myeloproliferative neoplasms (MPNs) including chronic myelomonocytic leukemia (CMML) and myelofibrosis (MF), acute myeloid leukemia (AML) (and potentially enriched in certain AML subsets), myelodysplastic syndrome (MDS), and chronic myeloid leukemia (CML). CD123 has also been reported on multiple myeloma (MM), acute lymphoid leukemia (ALL), hairy cell leukemia (HCL), Hodgkin's lymphoma (HL), and certain Non-Hodgkin's lymphomas (NHL). Important Safety Information from EU SmPC

Warnings and precautions

- Capillary leak syndrome (CLS), including life-threatening and fatal cases have been reported with most events occurred during the first five days of the first cycle of treatment. Before initiating therapy, it should be ensured that patients have adequate cardiac function and serum albumin ≥ 3.2 g/dL. During treatment, serum albumin levels should be monitored prior to the initiation of each dose, or more often as clinically indicated. Additionally, patients should be assessed for other signs/symptoms of CLS. Patients should be made aware of identifying CLS symptoms and when to seek immediate medical attention. Intravenous albumin supplementation and dosing interruptions may be required.

- Severe hypersensitivity reactions have been reported with ELZONRIS.

- Thrombocytopenia and neutropenia have been reported in patients treated with ELZONRIS monotherapy. The majority of events were reported in cycle 1 and cycle 2 of treatment, were not dose-limiting and did not recur in subsequent cycles.

- ELZONRIS can cause tumour lysis syndrome (TLS).

- Treatment with ELZONRIS has been associated with elevations in liver enzymes. Acute hepatic failure and liver encephalopathy has been reported in a patient treated with ELZONRIS at a higher dose (16 mcg/kg).

Summary of the Safety Profile

- The most serious adverse reaction that may occur during ELZONRIS treatment is CLS which was reported in 18% of patients with a median time to onset of CLS of 6 days.

- Adverse reactions occurring in ≥ 20% of patients treated with ELZONRIS were hypoalbuminemia, increased transaminases, thrombocytopenia, nausea, fatigue and pyrexia.

- Adverse reactions grade 3 and above according to the Common Terminology Criteria for Adverse events (CTCAE) and occurring in > 5% of patients were increased transaminases, thrombocytopenia and anaemia.
